A REDDITCH man killed himself despite being assessed by a doctor at the Alexandra Hospital as "low risk" of suicide, an inquest heard. 

Christopher Jan Srebniak, aged 53, killed himself in the lounge of his Plymouth Road home, a Worcestershire coroner ruled.

Over the weekend of April 9 and 10 Mr Srebniak began to suffer a breakdown in his mental health and indicated to his family that he had plans to kill himself.

On April 11 2016 he was taken by his family to his GP. 

The next day he was then taken to the accident and emergency department at the Alexandra Hospital in Redditch, where he was assessed by a doctor as being at "low risk" of suicide. 

In the early hours of the morning of April 13, he was discharged and went home, then, sometime before 8.15am, the same morning, he committed suicide. 

The medical cause of death is listed as a deep laceration to the neck and exsanguination, which is a severe loss of blood.

Chris Tidman, chief executive for Worcestershire Acute Hospitals NHS Trust, which covers the Alexandra Hospital, said: "As an NHS Trust we take mental health issues very seriously and during the inquest the coroner accepted the evidence given by our staff and did not criticise the care or treatment received in the A&E department at the Alexandra Hospital.

"This is a tragic case and our condolences go to Mr Srebniak’s family and friends."

An inquest into his death was opened by Worcestershire Coroners Court on April 18 and closed on Monday, August 8.
